Privacy policy: Verismo Terraform eForms extension
Effective October 24, 2026
Verismo Health Inc. ("Verismo", "we") runs Terraform, a library of certified eForms for clinics on Juno EMR, OSCAR Pro and Avaros, and the Verismo Terraform eForms extension for Google Chrome. This policy explains what the extension stores, what it sends to Terraform, and what it never collects.
What the extension does
The extension works only for clinics with a Terraform subscription, and the clinic's EMR address is its licence: when this browser opens the clinic's Juno, OSCAR Pro or Avaros, the extension turns on for that clinic by itself, with nothing to type. It then runs only on the clinic's EMR addresses and, for clinics whose plan includes pain clinic claim drafts, on ClinicAid (billing.clinicaid.ca). It turns on autofill and macros in Terraform eForms the subscription covers, shows which eForms are Terraform-certified and which version, fills a provider's signature stamp from the signature the clinic uploaded to Terraform, counts Terraform eForm submissions for the clinic's statements, and, for clinics with the Pain clinic plan, builds OHIP claim drafts and fills in the referring doctor's contact details on pain clinic letters.
What it stores in your browser
- The clinic's EMR addresses and its Terraform subscription status.
- This browser's own keys, created on the computer when the extension is turned on. The private keys cannot be exported and never leave the browser.
- Short-lived copies of Terraform's answers.
What it sends to Terraform
Everything goes only to Terraform at terraform.verismohealth.com, over HTTPS, signed with this browser's key.
- Turning it on: when this browser opens a Juno or OSCAR Pro page, that page's EMR address (only the address, e.g. https://yourclinic.secure.junoemr.com) is sent once to check whether the clinic has a Terraform subscription. If it has, this browser is turned on for the clinic with its public keys and a name such as "Chrome · macOS". Avaros serves many clinics from one address, so there the extension runs the clinic's "Terraform tenant check" report once, in the signed-in session, and sends its result: the clinic's tenant name in Avaros (e.g. novacrest), no patient data. An address or tenant without a subscription is not asked about again for a while.
- Opening a licensed Terraform eForm: the eForm's Terraform identifier, version and EMR type, and the EMR address it is open on.
- Downloading an eForm from the extension: the eForm's Terraform identifier and EMR type.
- Certification badges: an open eForm's content fingerprint (a hash of the form's design, not of anything typed into it), or, for an eForm list, the clinic's EMR address.
- Submission counts: when a Terraform eForm is submitted, its identifier, name and version, the EMR address, and whether it was a new or an edited form.
- Provider signatures: the signed-in provider's EMR provider number, to fetch the signature image the clinic uploaded to Terraform.
- Pain clinic claim drafts (Pain clinic plan, when someone presses the eForm's billing button): the billing fields of the pain clinic eForm, such as procedure and diagnosis codes and service dates, to build an OHIP claim draft. The note text and letters are not sent. The draft is passed to ClinicAid, the clinic's billing system, only when the user chooses to open it there.
- Referring doctor on a pain clinic letter (Pain clinic plan): when the patient's chart names a referring doctor but the EMR has no address for them, the extension looks up that doctor's billing number in the clinic's own ClinicAid session, as staff would on an invoice, and sends Terraform only the doctor's name and city, to find their practice address, phone and fax in Terraform's copy of the public CPSO register. Nothing about the patient is sent.
- On ClinicAid (Pain clinic plan): the extension runs on billing.clinicaid.ca in the tab the user opened from Juno. It reads the patient and provider names ClinicAid shows, to check they match the claim draft, then fills the draft's lines into the invoice and reads them back. It sends nothing about the patient or the invoice from ClinicAid to Terraform, and it never saves or submits a claim.
What it never collects
Apart from the pain clinic billing fields and referring doctor details above, the extension never reads or sends what is typed into an eForm. It never sends patient names, health card numbers, chart numbers, full page addresses (URLs) or browsing history, and it does nothing on websites other than Juno, OSCAR Pro and Avaros EMR addresses, Terraform, and ClinicAid (for the Pain clinic plan, as described above).
How we use it
Only to provide Terraform to the clinic: to check the subscription and certification, run the eForms, prepare the clinic's statements and billing for its subscription, keep the service secure, and support the clinic. We do not sell this information, use it for advertising, or use it to determine creditworthiness. We share it only with the service providers that host and operate Terraform for us, under confidentiality obligations, or where the law requires it. Health information in claim drafts is handled on behalf of the clinic.
How long we keep it
While the clinic's subscription is active, and afterwards only as long as needed for billing records and as the law requires. A browser that is turned off in the extension, or removed by the clinic, can no longer use the clinic's subscription.
Your choices
You can turn the extension off for a browser in its popup, or remove it from Chrome at any time. Pain clinic claim drafts are prepared only when someone presses the eForm's billing button. Your clinic's Terraform administrator can remove browsers from the clinic's subscription.
Changes and contact
We will update this page, and its effective date, before the extension starts collecting anything new. Questions or requests: support@verismohealth.com.
